Root Cause Questionnaire

Before our first session, I'd love to understand the fuller picture: your body, your environment, and the patterns underneath. Let yourself take your time with this; there's no need to rush, and there's no wrong way to answer. As you go, notice what comes up for you. That noticing is often as telling as the words themselves. Everything you share becomes the baseline we'll work from together.

Takes around 20 to 25 minutes.

Step 1 of 6

Section 01

About You

The basics, so I know who I'm meeting.

Section 02

Your Health History

The fuller picture: what's brought you here, what's been tried, and what's still unexplained.

Not the clinical version, the real one. How long, how it affects your daily life, what it's costing you.

Include physical, hormonal, and mental health, formal or suspected.

Beyond a diagnosis: how does your body actually feel day to day?

Practitioners, protocols, diets, supplements, programs. What helped, what didn't, what you've given up on.

Select all that apply.

Section 03

Your Baseline

A few honest numbers. This is where we'll measure change from, not to judge, just to know where you're starting.

This includes underwear and activewear.

Include brands where you can: deodorant, makeup, skincare, haircare.

e.g. do you live with housemates, family, alone, near industry or traffic?

Section 06

The Deeper Layer

The questions most practitioners don't ask. Share only what feels relevant and true.

Share only what feels relevant and comfortable. This isn't about having the right answer. It's about the fuller picture.

Relationships & Connection

Entirely optional. Only share what feels useful for our work together.

Looking Ahead

Be specific. The life, not just the body.

I can already see where we'll begin.

I'll read through everything before we sit down together, and a few things you've shared already have my attention. There's more happening beneath these answers than most people realize, and that's exactly what we're here to find. I'll see you soon.
